Greeting Parents of students at Bluff Creek Elementary,

Eastern Carver County Schools is very happy to announce they are hosting their first ever FREE dental clinic, Adopt A Smile, at Chaska High School January 27-31, 2020.  The program is sponsored through Operation Grace.  Anyone without dental insurance will qualify!  You may print the attached forms, both in English and Spanish.

Please be sure to complete all paperwork, including answering all necessary questions, and providing all signatures.   Return paperwork to the BCE Health Office, deadline is December 20th.  If any part of the paperwork is incomplete, they WILL NOT see your child.

Once forms are turned in, a schedule will be put together.  Your student will be notified of his/her date and time to come to the clinic. Transportation will need to be provided by the parent/guardian.

The process:

  1. Students are placed in a schedule to see a hygienist.  They will clean teeth, provide fluoride and sealants and do x-rays (if indicated).
  2. Students that need further care will be set up to see a Dentist in the weeks that follow.  This will also take place at Chaska HS.  All paperwork for that visit is included in the original packet, so be sure to complete all the forms.

VOLUNTEER OPPORTUNITY!

We are looking for a couple of parent volunteers to be “runners” in the school the days of the clinics to get the students to their appointment on time.  Please contact Nurse Gina, in the CHS Health Office if you’re able to help with this, 952-556-7119 or Skluzacekg@district112.org

Also, if you or someone you know works in the Dental Profession, please reach out if interested in volunteering.  Many times these free clinics are staffed by people in the local community with close ties to the area.
